Registration

How do I enroll in ThinkWellness?

In order to enroll in ThinkWellness, your account must be setup with Human Resources.

Go to the Registration page to start the enrollment process.

If you have difficulty registering, please contact Human Resources for more information.

How do I find my employee ID?

Your employee ID can be found on your pay stub PDF.

If you have difficulty finding your employee ID, please contact Human Resources to receive it.

How does my spouse enroll?

In order for a spouse to enroll, their account must be created by Human Resources.

When a spouse goes to enter an employee ID, they must enter the employee ID of the employee followed by a "-S" (i.e. 0123-S).

Go to the Registration page to start the enrollment process.

Activities

How do I complete an activity?

Go to the Activities tab and select an activity.

Review the requirements of the activity and then press "Start Activity."

Once the activity is started, print the document (Ctrl+P / ⌘+P or press the Print Form button).

Follow the provider instructions listed on the page.

Once the provider sheet has been completed, there are two options:

  • Take a clear, complete picture of the form or scan it, and upload it at the bottom of the activity page.  Ensure that the scan code on the form matches the code on the activity page.
  • Return the form to Human Resources.

Human resources will review the activity and mark the activity as completed.

If there is an issue with your form, Human Resources will contact you with more information.

What information is needed on my activity form?

In order to minimize the sharing of personal information, please do not place your name or other identifying information on the provider form.  The scan code on the form will identify you within ThinkWellness for activity completion.

The form with the scan code must be uploaded or returned to Human Resources for the activity to be completed.
